Authorized Dealer Dash X FM Return and Replacement Request Form
Please fill in your contact info, along with the serial number of all units in stock, so that we can be sure and coordinate the appropriate return method for the Dash X FM affected units. Upon receipt of the form, the Burley customer service team will follow up to provide you with instructions for returning your Dash X FM units, as well as getting replacement units sent out to you.
If your in-stock bike seats have not been recalled, no further action is required.
Note: This recall only affects the Dash X FM child bike seat for specified Lot #s. The recall does NOT apply to the Dash FM or the Dash RM.
